Faith is not in network with any insurance plans. A Superbill can be provided to you upon request, which you may submit to your insurance plan to utilize any Out Of Network Benefits.

Due to the No Surprises Act (NSA) beginning January 1, 2022, if a client choses to utilize Faith’s services as Out Of Network by the client’s choice, the appropriate Good Faith Estimate of services will be provided and proper paperwork and disclosure will be completed.
